Most people understand the connection between smoking and diseases such as lung cancer, heart disease and stroke. But researchers have also been studying another important question: What impact might smoking have on prostate cancer?

The answer is more complicated than simply saying smoking causes prostate cancer.

Research has not established the same clear relationship between smoking and developing prostate cancer that exists with several other smoking-related cancers. However, evidence has increasingly linked smoking—particularly long-term or heavy smoking—with more aggressive prostate cancer and poorer outcomes among men diagnosed with the disease.

That distinction matters.

For men thinking about prostate health, smoking is another factor worth discussing with a healthcare provider—along with age, family history, screening history and other individual risk factors.


What Does the Research Show?

Studies examining whether smoking increases the overall likelihood of developing prostate cancer have produced mixed results.

The evidence becomes more concerning when researchers look beyond whether prostate cancer develops and examine how the disease behaves and what happens after diagnosis.

Research has associated smoking with:

A 2025 scientific review of smoking and prostate cancer concluded that smoking—particularly long-term and heavy use—deserves greater consideration as a modifiable risk factor while also emphasizing that more research is needed to fully understand its effect on prostate cancer development and progression.

The takeaway isn't that every man who smokes will develop prostate cancer.

It's that smoking may matter when we talk about how prostate cancer develops, progresses and affects men over time.


Smoking and Aggressive Prostate Cancer

One of the more concerning findings comes from research involving men who had already been treated for prostate cancer.

A study highlighted by Harvard Health followed more than 22,000 men who had undergone surgery for prostate cancer.

Compared with nonsmokers, prostate cancer patients who smoked had:

89% higher risk of dying from prostate cancer

151% higher risk of their cancer spreading

40% higher risk of their PSA rising again after surgery

A rising PSA following prostate cancer surgery can be an indication that cancer has returned.

These numbers don't prove that smoking alone caused the poorer outcomes. Many factors can affect an individual's cancer progression and survival.

But the findings contribute to a larger body of evidence associating smoking with more aggressive disease and prostate cancer mortality.


Developing Prostate Cancer vs. Prostate Cancer Outcomes

This is an important distinction.

Does smoking cause prostate cancer?

The relationship remains uncertain.

What about men who already have prostate cancer?

Multiple studies have associated smoking with increased prostate cancer progression and mortality.

A 2025 review cited meta-analytic evidence showing higher prostate-cancer mortality among current smokers compared with nonsmokers. The review also notes that smoking may influence both biological processes and health behaviors related to prostate cancer.

In other words:

The question isn't only whether smoking contributes to developing prostate cancer. It is also whether smoking may influence what happens after prostate cancer develops.

Both questions deserve attention.


Why Might Smoking Affect Prostate Cancer?

Researchers are still investigating exactly how smoking may influence prostate cancer.

Cigarette smoke exposes the body to numerous chemicals and toxic substances. Although smoke enters through the lungs, those substances and their effects aren't necessarily limited to the respiratory system.

Oxidative Stress and DNA Damage

Cigarette smoke contains substances capable of damaging cells and DNA.

Smoking can also contribute to oxidative stress—an imbalance between harmful molecules known as free radicals and the body's ability to neutralize them.

Over time, cellular and DNA damage may contribute to processes involved in cancer development or progression.

Hormonal Effects

Researchers have also investigated whether smoking can influence hormones and other biological processes that may affect prostate cancer.

What Men Can Do

You can't control every prostate cancer risk factor.

You can't change your age.

You can't change the family you were born into.

And you can't change your genetic history.

But some health behaviors can be changed.

If you currently smoke:

Talk with your healthcare provider.
Discuss your smoking history as part of your overall health and prostate-health conversation.

Know your prostate cancer risk.
Age, family history and other individual factors can influence when conversations about prostate cancer screening should begin.

Don't wait for symptoms.
Early prostate cancer may cause no noticeable symptoms. Talk with your healthcare provider about whether PSA testing or other screening is appropriate for you.

Ask for help quitting.
Stopping tobacco use can be difficult, but healthcare professionals can help identify evidence-based cessation options and support.

If you've been diagnosed, include smoking in the conversation.

A Modifiable Risk Factor

Some prostate cancer risk factors are beyond our control.

Smoking is different.

That doesn't mean quitting guarantees that a man won't develop prostate cancer or that his cancer won't progress.

It means smoking is something a person can potentially change.
